Investigation Summary

Investigation Nr: 200651362
Event: 09/05/2007
Employee injured when pinned by forks of lift truck

At approximately 9:00 a.m. on September 5, 2007, Employee #1 was using a four-point lifting strap attached to a Thomas 233 skidsteer lift truck to raise a trowel onto a trailer. The straps had to be kept tight to keep the hooks in place, so Employee #1, who was standing between the skidsteer and the trowel, was holding them tightly as he reached into the cab of the lift truck to position the forks. He mistakenly pulled on the DOWN lever, lowering the forks and pinning himself against the vehicle. Employee #1 suffered a fractured leg and pelvis, for which he was hospitalized.
