Investigation Summary
On January 22, 2004, Employee #1, a fork lift operator, was standing on the front of the lift to reach a 2-by-4 spacer that he was attempting to reposition on a pallet of wood before lowering the load that he was carrying when he fell into the hand controls causing the mast to move back toward the cage. He was caught between the mast and the cage and was killed.
