Investigation Summary
On May 17, 2003, Employee #1, a truck driver, had delivered a load of floor grating and I-beams to a site, un-strapped the load, and directed forklift operators to begin unloading. As one operator lifted the load of flooring, the tip of the right fork caught a beam, causing it to fall off and strike Employee #1 (who was on the other side of the trailer) in the waist and upper legs. He was hospitalized with fractures.
