Employee Fatally Injured When He Fell From Aerial Lift

On August 21, 1999, Employee #1 fell from the bucket of a hydraulic aerial lift. Employee #1 was fatally injured when the hydraulic aerial lift malfunctioned and dropped fifteen feet causing Employee #1 to fall from the bucket. Employee #1 was not wearing a safety belt.
