Investigation Summary
At approximately 11:30 a.m. on March 1, 2017, an employee was adjusting a pipe. He was using a chop saw to cut a pipe and the pipe was wet, which caused his hand to slip into the saw blade. The blade amputated the first joint of his left index finger. The employee was hospitalized and treated for his injuries.
