Investigation Summary
Employee #1, a tunnel operator, was clearing a fault from the second stage of a Milner press, model MP603IL, serial #AAA/6139280. He was reaching under the bell of the press when the machine cycled, pinning him inside. Employee #1 was crushed and died as a result of traumatic asphyxia.
