Employee's fingers are amputated during troubleshooting

At 8:45 p.m. on March 4, 2017, an employee was troubleshooting a paper jam that had occurred. After performing the necessary lockout/tagout procedures, the employee used on of his hands to pry apart two parts of the machine that had become stuck together. The force of the two parts separating caused the amputation of two fingers on his right hand.
