Investigation Summary
At 4:15 p.m. on February 28, 2017, an employee was working for a finish carpentry contractor. He was making a wooden template and he was doing this by performing a plunge cut with a dado blade installed on a table saw. The piece of wood being cut was kicked back. When this occurred, the employee's left hand struck the saw blade. Part of the middle finger on his left hand was amputated and he was hospitalized.
