Investigation Summary
At approximately 8:00 p.m. on February 24, 2017, Employee #1 was oiling dies and placed his finger in between the dies to oil them in between the cycle of parts. A coworker cycled the machine and Employee #1's finger was caught in between the dies. Employee #1's finger was amputated but he was not hospitalized.
