At 12;54 p.m. on January 25, 2017, Employee #1 was unjamming the conveyor system and didn't clear himself from the conveyor when a coworker turned on the conveyor system. Employee #1's right arm was crushed from the top of the wrist to the bicep. He was hospitalized.
