Investigation Summary
At approximately 10:30 a.m. on February 23, 2017, Employee #1 was greasing a mill drum and trunnion. This procedure was performed with the drum running so the contact area gets completely covered. Employee #1 tried to grab some grease that had dropped on the ground, lost his footing, and three fingers on his left hand became caught between the trunnion and the drum. Employee #1 was hospitalized with three amputated fingers.
