Investigation Summary
At approximately 6:30 a.m. on February 23, 2017, Employee #1 was walking from a parking lot when he was struck by a Nissan forklift that was operating in the forward direction with a load. His left foot became wedged between the pallet and the floor causing him to fall and roll approximately 3 feet away from the forklift. He sustained fractures to his left ankle and shoulder and was hospitalized.
