Investigation Summary
At 1:30 a.m. on February 27, 2017, and employee was cleaning the pour hole to remove molten metal build up from a die machine. The machine cycled, and one of the employee's middle fingers became caught between the ram and die. He sustained an amputation of his middle finger, which he was hospitalized and received treatment.
