Investigation Summary
At approximately 11:00 a.m. on February 17, 2017, an employee was going to remov e a bleeder valve from the skid. When the employee removed the plug high pressur e exceeding 1,100 psi, shot out and caused the employee to be blown back. The em ployee sustained injuries to his leg (puncture), a broken left arm, and skull fr acture to both sides of his face. The employee was hospitalized and treated for his injuries.
