Investigation Summary
At 12:15 p.m. on January 11, 2017, an employee was cleaning a flat belt on the 6FX1 gluing machine while it was running. The employees rag was caught between two pulleys and the rag pulled his right thumb into the pulleys. The employees right thumb was lacerated by the pulleys. The employee was not hospitalized.
