Investigation Summary
At approximately 3:00 a.m. on February 14, 2017, Employee #1 was cleaning the mixing auger of a machine. Employee #1 stuck his hand into a drain port on the machine and the rotating auger amputated three fingertips. Employee #1 was transported to a medical center, where he underwent surgical procedures and received addition treatment, and was then hospitalized.
