At 1:00 p.m. on February 14, 2017, an employee was installing prefabricated metal formwork for cast-in place concrete construction. As he climbed about 5.33 feet of the vertical formwork wall, where he fell and sustained bruising on his back. The employee was hospitalized and treated for his injury.
