Investigation Summary
At 2:00 a.m. on February 5, 2017, an employee was cleaning parts on a piece of equipment. As he worked to remove material that was caught in the shaft of a conveyor, the employee's left arm became caught in the exposed rotating shaft. The employee was taken to a nearby medical facility, where he was hospitalized and treated for multiple fractures and crushing injuries to his left arm.
