Investigation Summary
At 3:00 p.m. on February 2, 2017, an employee was removing a threaded plug from the top of a 55 gallon drum while machine was in movement, and talking to a coworker simultaneously. His left ring finger became caught in the machine, resulting in its amputation at the first knuckle.
