Investigation Summary
At approximately 1:30 p.m. on September 24, 1993, Employee #1 was grouting brickwork in a manhole. Nearby, a white GMC water truck was backing up. Employee #1 attempted to get out of the way but was struck by the driver's side rear wheels and was knocked to the ground. The truck wheels landed on Employee #1. Employee #1 sustained multiple injuries to his right side and was hospitalized.
