Investigation Summary
At 9:30 a.m. on January 26, 2017, an employee was feeding fish tape through a 3 inch pipe above an energized buss. Either the fish tape or another unknown object contacted the energized buss, causing an arc flash. The employee sustained second degree burns to hand, forearm, and face.
