Investigation Summary
Employee #1 was working beside the stationary metal conveyor rack, bunching together 1/4 in. by 20 in. steel. The motorized conveyor was traveling south when it struck Employee #1 and crushed him against the metal conveyor rack. The audible alarms on the motorized conveyor were not functioning at the time, and there was no horizontal clearance between the stationary rack and the motorized conveyor.
