Employee killed when struck by bundle of boxes
Employee #1 was in a box storage area stacking empty, flattened bundles of cardboard boxes. He had stacked four bundles, which stood approximately 14 ft high. Employee #1 was on the floor in front of the stack when the top three bundles fell, striking and killing him.
