Employee crushed and killed in forklift overturn

Employee #1 was using a forklift to load a flatbed wrecker truck. Apparently, the flatbed was not secured by wheel chocks. As Employee #1 attempted to drive the forklift onto the flatbed truck, the truck suddenly moved forward. The forklift fell between the flatbed truck and the back of a tractor trailer, crushing and killing Employee #1.
