Investigation Summary
At 9:00 p.m. on January 24, 2017, an employee was performing preventative maintenance on the running conveyors in the warehouse by greasing the chains while on the scissor lift 20 feet high. The employee's rag got caught in the chains, pulling the employees fingers in. The employee received partial amputation of three fingers.
