At 12:00 p.m. on January 18, 2017, Employee #1 was cutting carpet using the #1 cutting machine. The employee opened the door on the guard of the machine to adj ust the carpet. When he reached into the unguarded blade area to adjust the carp et, the blade came back and amputated the end of his finger.
