ELECTRIC SHOCK & FALL - DIRECT CONTACT WITH ENERGIZED PARTS

A maintenance supervisor was working from an aluminum ladder, repairing electric equipment. The equipment had not been deenergized. The employee contacted an energized part, received an electric shock, and fell from the ladder to the concrete floor. He died either as a result of electrocution or as a result of injuries sustained in the fall.
