Employee's skull fractured when struck by chain

Employee #1 was helping to change rolls on a paper machine. Each roll was 12 inches in diameter and 20 feet long and weighed approximately 1,500 lb. Two chain-falls, along with a one-hand operated come along, were being used to lift the new roll in position. The chain-fall on the side where Employee #1 stood slipped. The block on the chain-fall struck Employee #1 on the right side of his head and pushed it into a nearly 1 inch thick steel mounting plate. He received a slight skull fracture.
