Investigation Summary
Employee #1 was operating a Delta 48 inch tension slitter. The tension slitter takes a coil of metal, of different thickness and width, and slits it into whatever width a customer wants. On the day of the accident, a new slitter head had been installed. Employee #1 was starting up the machine when his left arm was somehow caught between the slitter rolls and was pulled in up to his elbow. Due to damage, Employee #1's left arm was amputated at the hospital. The nip point between the rolls had a guard, but it was not in place at the time of the accident.
