Employee crushed and killed by front end loader

At approximately 7:00 p.m. on January 27, 1995, Employee #1 was found caught between the bucket and the cage of a front end loader. The loader motor was running. Employee #1's head was crushed. He was taken by ambulance to the hospital, where he was pronounced dead. There were no witnesses to the accident.
