Employee killed when head crushed between forklift and rack

At approximately 10:00 p.m. on November 14, 1993, Employee #1 was operating a Clark stand-up forklift. It appears that as the employee turned the forklift, his head was caught between the upright bar of a storage rack and a vertical bar on the rear of the forklift. Employee #1 was killed when his head was crushed.
