Investigation Summary

At 8:45 a.m. on May 25, 1994, Employee #1 was working on Highway 35W in the northbound lane as part of a work crew that was boring under a highway to lay casing for fiber optic cable. He was marking the direction and depth of the boring machine head by spray painting the highway surface where the boring head was located. Employee #1 was struck by a Toyota 4x4 truck, which amputated his left leg above the knee and caused other serious injuries from which he died at the county hospital. He was wearing a reflective vest and hard hat and was working in the traffic control work zone, which had all the temporary warning signs situated before the actual work site.
