Investigation Summary
At approximately 3:50 p.m. on April 6, 1994, Employee #1, an operating engineer/foreman, was hand signaling a crane operator who was picking up a section of a roof at a demolition site. Employee #1 was on the structure's second floor near a barred window. The roof sections had been previously cut and the section to be picked was secured at all four corners by wire-rope chokers. Five successful picks had been completed. The adjacent bay had one middle section removed (fifth pick), and the sixth pick was about to be made. Employee #1 apparently felt safe standing next to an 11 to 12 ft brick wall. He was approximately 22 ft from the section to be picked. There was nothing directly overhead. The crane picked up the sixth section 1 or 2 ft. The seventh section to be picked was still partially connected to the sixth section and was pulled along with it. The seventh section fell free of the brick wall and fell to the second floor, pulling the parapet of the brick wall over with it. The parapet collapsed the entire length of the wall, which struck and crushed Employee #1. He died instantly of multiple injuries.
