Investigation Summary
On July 30, 2003, Employee #1 set a 12-ft metal extension ladder on top of a 10-ft rolling scaffold. He placed the ladder against the wall and ascended it to perform some electrical work. The ladder caused the scaffold to tip over and Employee #1 fell approximately 16 ft to the cement floor. He sustained a fractured shoulder and was hospitalized.
