Employee injured in fall from ladder trying to access roof

On September 17, 2004, Employee #1 placed a metal extension ladder against the chimney at the back of a house to repair a facia board. He climbed to the top of the ladder and, keeping his left foot on the top rung, swung his right foot around and onto the roof. Employee #1 was attempting to pull himself up onto the roof when he apparently lost his balance and fell approximately 18 ft onto a cement surface. He sustained injuries that required hospitalization.
