Investigation Summary
At approximately 8:45 a.m. on June 24, 2003, Employee #1 went to the tool warehouse to look for a small welding machine. He asked a coworker about the welder but he did not know anything. Employee #1 thought it may be on a pallet stored up on a metal storage rack. He did not have a ladder available. There were many pallets of tools in the warehouse and it would take at least 2 hours to get to the rack with a forklift. So, he decided to climb up and look for the welder. While standing on the rack, he looked in one pallet of material but did not find the welder. As Employee #1 was moving to the next pallet, he fell off of the storage rack onto the concrete floor. Employee #1 was taken to Mercy San Juan for the initial care and later to Roseville Sutter. He broke his (L) knee cap and patella and his (R) wrist, and sprained his left arm.
