Investigation Summary
On August 12, 2003, Employee #1 was working alone at the company's vine seed cleaning facility. While he was washing the vine seed, a metering valve located at the base of a storage hopper became jammed with plant debris. The employee was clearing the jam with a water hose and he placed his right hand into the valve's narrow discharge chute, resulting in his fingers becoming entangled in the rotating valve. The tip of his fourth finger was amputated and his third finger was fractured. He was transported to Colusa Community Hospital where he was hospitalized with his injuries.
