Investigation Summary

Investigation Nr: 201174034
Event: 09/04/2003
Employee injures finger in machine

On September 4, 2003, Employee #1 was working as a bundler in the tail-off area of a paint line in a molding plant. He reached 22 in. into a barcode roller to remove what he thought was barcode tape wrapped around the metal shaft. The barcode roller was powered by a 0.5-HP, 1,700-rpm electric motor that was running. What Employee #1 thought was paper tape was actually nylon banding material that was wrapped securely to the shaft. The tip of his right middle finger was amputated and he was hospitalized at Enloe Hospital.
