Investigation Summary
At approximately 2:01 p.m. on November 30, 2003, Employee #1, a roofer, was removing 2-ft by 4-ft-crawl boards from the OSB sheeting, in preparation for applying roofing felt paper, when he slipped and fell from the roof, striking the top rail of a scaffold that was about 3 ft below the eve of the roof as he fell. The eve height was 15 ft by 8 in. Employee #1 received first-aid from his coworkers before paramedics arrived to continue treatment. Employee #1 was taken to the hospital where he died.
