Conveyor belt Injures Employee

On January 24, 2004, Employee #1 was part of crew that was feeding 6-in. poly liners through a Pinch Roller (Wet-out conveyor). This equipment consisted of an inclined belt conveyor that took the liners up to a roller that would squeeze the liners to a pre-set thickness. Employee #1 walked up the moving belt conveyor and tried to climb over the backside of the equipment to enter a trailer parked at the rear of the conveyor. As Employee #1 tried to climb over, his left foot made contact with the moving belt conveyor and he was drawn into the roller assembly. Employee #1 sustained fractures to all five toes and required surgery to insert pins into his big toe. Employee #1 was hospitalized. At a later date Employee #1 had to undergo amputation of two injured toes.
