Investigation Summary
At 2:40 p.m. on January 11, 2017, an employee was tapping on a valve to get it to turn so he could replace it. As the employee turned the valve, it dropped off and struck the employees right leg directly below his knee, which had been previously replaced. The employees leg broke a couple of inches below where the knee had been replaced when struck by the valve. The employee was hospitalized.
