Employee breaks water fallf legs when load fallform forklift

On May 19, 2003, Employee #1 was operating a flatbed and after the last load of plywood was unloaded, the truck driver walked over to the flatbed to retrieve 2 ft by 4-ft dunnage. The load of plywood struck the employee on the right shoulder, knocking him to the ground and the load fell on his legs. Employee #1 broke both legs when the load of plywood (roof packs) fell off the forklift, and struck him. Employee #1 was hospitalized for 15 days.
