Employee injured in fall from ladder

Employee #1, working on the second floor walkway, was installing a duct into an opening above the wall. He was standing on an A-Type 8-foot ladder that was in the closed position and leaned up against the wall. He climbed up the ladder and was lifting the duct over his head when he lost his balance and fell backwards over the walkway's guardrail. He fell 25 feet to the first floor, sustained serious back injuries, and was hospitalized.
