Investigation Summary
Employee #1 was feeding corrugated sheets into a slitter located in the Water Jet Department when a sheet became stuck in the roller. The employee attempted to pull the sheet out when his right arm was caught in the roller and pulled into the machine. He sustained broken bones and tissue damage to his right arm and hand and was hospitalized.
