Investigation Summary
At 2:18 p.m. on September 7, 1994, Employee #1 and a coworker were working in a 3 ft trench next to an underground section of wall that was thought to be doweled to the foundation. A 5 ft section of the 3 ft by 6 in. wall fell and landed on Employee #1 and the coworker. Employee #1 was taken to the hospital and released with minor bruises and a slight back strain.
