At 11:27 a.m. on August 25, 1994, Employee #1 was attempting to activate a 440V fan with a motor controller switch. A rodent in the controls crossed the contacts, resulting in an explosion and fireball. Employee #1 suffered 1st- and 2nd-degree burns on his arms and face. He was hospitalized.
