Investigation Summary
Employee #1 was crossing from a deli display to a walk-in refrigerator in the bakery as another employee was doing the daily cleaning of the bakery floor that is required in the food preparation area for hygiene reasons. Employee #1 knew the floor was wet, but she was not aware of the amount of cleaning agent used, which made the floor slick. She slipped and fell, sustaining muscle/back injuries from the fall and chemical burns from contact with clothing dampened by the alkaline cleaner. Employee #1 was sent home, approximately five minutes away from work, to shower and change clothes. Employee #1 experienced pain while she was showering, and she was taken to the hospital two minutes away. Employee #1 was treated for several hours in the emergency room and then released. Employee #1's chemical burns healed within two to three weeks, but she is still receiving physical therapy for fall-related back problems. MSDS was not available at the time of the accident. The injury was not the result of a reportable accident.
