Employee falls as ladder collapses

At approximately 8:45 a.m. on August 2, 1994, Employee #1 was attempting to step from a ladder onto the roof of a condo unit. The ladder collapsed and he fell approximately 18 feet to the ground. Employee #1 fractured his right heel and bruised his tailbone. He was transported by paramedics to Tahoe Forest Hospital, where he was treated and released. Employee #1 stated that the ladder was not tied off. He felt that the holding supports for the extension failed. The ladder was in good repair but the holding devices did not completely cover the rungs; they were only held by approximately one inch.
