Employee's hand lacerated when machine not locked out

At about 10:00 p.m. on August 19, 1994, Employee #1 of Campbell Soup was one of two operators working at packaging line #8. He noticed that the adhesive tape on the case sealer was not being cut off properly, and went to clean the cutting knife. He turned the machine off, using only the OFF button, and began working. The co-operator on the machine, seeing that Employee #1 had not locked out the controls, went over to help and began to lock out the controls. She accidentally hit the ON button, starting up the machine, which pulled Employee #1's hand through the machine and over the knife blade between several cases of soup. First aid was given and Employee #1 was then transported by ambulance to Sutter General Hospital in Sacramento, CA. He was hospitalized for a deep laceration to the left palm and the removal of the skin and flesh on three fingers of the left hand.
