Investigation Summary
Employee #1, a cleanup man at a slaughter house, was cleaning a conveyor table in the kill room using a high pressure hose. He removed a guard to clean the guard and the chain and sprocket drive. He then proceeded to clean underneath the table before replacing the guard. His glove became caught in the chain, pulling his hand into the sprocket and amputating his left finger to the first knuckle. Employee #1 was performing his usual and customary duties.
